Requirements and Conditions
New Prescription:
It will be necessary for you to fax or mail in your prescription. It is also acceptable for your doctor to call in a prescription.
Transfer Prescription:
It will be necessary to provide us with the name and phone number of the pharmacy that has filled it for you in the past and your doctor’s name and phone number.
Refill Prescription:
Simply let us know you would like a refill of a prescription currently on file or complete the form below.
Vacation Prescription Refills:
We can ship to you anywhere in the USA when you are on vacation. Please allow time for compounding and shipping.
